How to Enroll in 4-H:

4-H is open to youth in grades kindergarten through 13 (one year out of high school).  As part of the enrollment process each member is required to sign and submit a behavior guideline form.  This form provides 4-H members with guidance for appropriate behavior when participating in activities and events.  Click on the buttons below for the enrollment forms, behavior guideline form, and the directions to fill out the enrollment forms.  If you have questions or for further 4-H information, you can contact us in the Lincoln County UW-Extension Office at (715) 536-0304.
