Secretary/Club Record Book

It is very important that club secretary, treasurer, reporter and the club leader keep accurate club records. The following are links to the forms contained within a Club Record Book which is distributed to the clubs in September, at the beginning of the 4-H year. Each club should turn in their club record book in September by the designated deadline.  All links lead to the Wisconsin state 4-H Youth Development site that has the most recent information and forms.   Any county specific forms are downloadable from this page.

Yellow covers used to keep required forms and records for the 4-H club secretary, treasurer, reporter and organizational leader.  The Extension office has copies on yellow cardstock paper.

Calendar of Club Activities

It is important for 4-H clubs plan fun and effective club meetings that have a balance of recreation, education and business. Club Attendance Record

Even in small clubs, it is important to keep an accurate record of member attendance.

Membership Summary Form -- Club

Form for clubs to record information on youth members, including name, address, parents, grade, year in 4-H and current projects.  The roster that the Extension office provides may be substitued for this form.

Junior Leader Summary Record

Learning leadership skills is an important part of the 4-H experience.  .

Club Attendance Record

Even in small clubs, it is important to keep an accurate record of member attendance.  The UW Extension office can provide each club with a computer printout of enrolled members.
