Lincoln County 4-H Accident/Injury Report Form
Volunteers are asked to complete this form and return to the UW-Extension office as soon as possible following any accident/injury that occurs at a 4-H event.

Permission to Particpate in 4-H Activity
When a club or project group is conducting a field trip, tour, or other outing, activity consent forms should be distributed to members. This form explains the event and gets parental approval for the youth to participate, medical information and permission to seek care in case of accident/illness. The top half of the form stays with the parent, while the bottom portion is returned to the adult supervisor prior to the event.

Volunteers Driving
Whenever possible try to utilize bus transportation versus asking volunteers to transport youth. When clubs or committees have adults transporting 4-H members, other than their own children, they are encouraged to have the driver complete an insurance information card and to keep it on file. This information indicates to the person organizing the event that the driver has auto insurance. Persons without coverage should not transport other family’s children.
